Improved from what?
Without a useful starting picture, later change is difficult to interpret. A strong lesson may produce impressive work, but if we never established what the learner could do beforehand, we may not know which part changed.
Baseline Before Intervention asks what the learner could do, independently and under stated conditions, before the teaching or repair began.

A later improvement claim becomes more credible when the starting state was observed clearly enough to show what was already present, what was missing, and what support was available before intervention.

English: Preserve What the Learner Already Understands
An English learner may understand a passage accurately but write a weak answer. A baseline should separate meaning, evidence selection, structure and language control before intervention begins.
If oral meaning is already strong, the repair should not pretend comprehension itself is absent.

Mathematics: Find the Earliest Weak Link Before Rebuilding
A Mathematics learner may represent the problem correctly but select the wrong route; or select the right route and lose accuracy during execution.

Voyage: Baselines Change as Responsibility Moves Inward

Does every lesson need a formal baseline?
No. A brief independent attempt, prior work sample or focused question may be enough for the educational decision being made.
What if the learner is too stuck to show much?
That itself is evidence. Record where orientation stops, then begin teaching. Baseline should not become prolonged frustration.
